  • Record your glucose levels in the space provided.
  • Record your medication dose, especially any changes in your medication.
  • In the comment section, record changes in food, activity, illness, stress, or insulin reactions.
  • Bring your logbook to all your appointments with your diabetes healthcare providers so they can assess your diabetes control.
  • Always use your logbook even if you're traveling or are on vacation.
